Documentation Update Manifests

Each Documentation Helper execution that successfully or partially processes a completed round must create one manifest file here:

- `GP-DOCUP-000123.json`

The manifest must reference:

- target round

- helper execution ID

- status

- documents examined

- documents created/modified

- sections changed

- source evidence

- commits

- unresolved questions

- resulting hashes

- validation results

- revision/supersession relationship where applicable

Do not create duplicate successful initial manifests for the same completed round.
